• Immunization Requirements for School Entrance 26-27 – This chart depicts the immunizations that are REQUIRED for your child to begin school for the 2026-2027 school year
• NYS Health Examination Form – REQUIRED Form - to be completed by a private health care provider or the school medical director
• New Student Health History Form – to be completed for new entering students
• Dental Health Certificate – Although not required, this certificate is incredibly helpful in maintaining your student's dental health
• Medication Form – This form must be completed and signed by both a parent/guardian and your student's primary care provider before your student may receive medications in school.
• Emergency Medication – Self Carry Form – This form is required in order for a student to independently use and carry emergency medications.

Cherry Valley-Springfield Central School has a School Based Health Center located in our building.  This is a partnership between CV-S and Bassett Healthcare Network. A SBHC provides comprehensive healthcare services to students in pre-kindergarten through 12th grade at the school. The SBHC can become your child’s main healthcare provider and is able to take care of all primary health needs, even when school is not in session.
